Academic Activities Approval

Complete with courses you wish to add/drop, seek approval and signature from your Program Director, enrol courses in ROSI, forward the form to the Registrar's Office for record. (Doctoral students are not required to complete this form but it will be the student's responsibility to seek approval from your Doctoral Supervisory Committee regarding the appropriateness of the courses.)

Financial Assistance

Complete and forward under confidential cover to the Registrar's Office before the deadlines.

Graduation Application 2014

Complete and forward to the Registrar's Office before the due date for submission.

Request for Letter

Complete and forward to Registrar's Office.  Allow five business days for issue.  Students must have paid fees and registered in the current and at least one immediate upcoming terms.

Re-Registration After 'FINCA'

Complete and make an appointment to see the Student Services Assistant.

Transcript Request

Complete and forward with cheque payment to the Registrar's Office.  Allow at least five business days for processing.

Verification of Student Illness or Injury

Attach this form with request for special academic consideration based on illness or inujury

Withdrawal from Program

Complete and make an appointment to see the Registrar. 

For AD students:

AD Declaration of Full-Time / Part-Time Status

Complete and forward to Registrar's Office by August 31 (D.Min. by March 31).  This form has to be completed each year for your study at the College.

AD Reading and Research Course Proposal

Complete form with your instructor, seek approval and signature from your Program Director, forward to Registrar's Office before the due date for adding courses for each session (see "Important Date" for due date). 

AD Request for Extension to Complete Course

Professors may grant extensions to individual students allowing them to submit work by a specified date after the prima facie deadline. Individual extensions cannot be granted beyond the end of the academic year (i.e., beyond the end of August). The TST AD Director can grant an extension beyond August 31 up to the absolute deadline on the recommendation of the faculty member and the Advanced Degree Director. To request an extension, a student needs to complete and submit the Request for Coursework Extension Form.

AD Request for Extension to Complete Program

Students wishing to register beyond the sixth year of their program are required to submit this form at least three months prior to the beginning of the next session.  Extensions requests can be granted twice, each time with a maximum of one calendar year.

AD Request for Leave of Absence

Leave of absence is not recommended by the College unless under extenuating circumstances.  Complete and forward to Registrar's Office. Attach a doctor's note if requesting for a sick leave.

Audited Courses for AD Students

Complete and forward to the Registrar's Office prior to the first class meeting and after completion of the course, with required signatures as specified on the form.

ThD/PhD Request for 1st Extension to Complete Comprehensive Exam

Doctoral students not able to complete their Comprehensives within three years from their date of admission to the program are required to complete this form. Students are recommended to submit this request at least three months prior to expiry of the 3-year period.

THD/PhD Request for 2nd Extension to Complete Comprehensive Exam

A second extension may be granted. Students are recommended to submit this request at least three months prior to expiry of the 1st Comprehensive Extension due date.

TST: Exemption of Language Requirement

Please contact the AD Director regarding exemption. Submit form via the College Registrar's Office.

TST: Thesis Proposal Form

Please contact your Committee Chair. Submit form via the College Registrar's Office.

For BD students:

BD Request for Request to Complete Course

Please have this form signed by your instructor, and forward it to the Registar's Office before the last day of the examination week for each session. Please note that "SDFs" will affect enrolment in the subsequent academic term(s).

Choice of Academic Advisor (for M.Div. and MTS students)

Complete and forward to Registrar's Office by January of your first year of study.  Complete a new form if Academic Advisor has changed.

Choice of Program Mentor (for MA in MS students)

Complete and forward to Registrar's Office by January of your first year of study.  Complete a new form if Academic Advisor has changed.

For International Students:

Notice of Collection of Personal Information and Consent

Complete and return to the Registrar's Office with your confirmation of acceptance. Receipt of this form is required to proceed and retain student's enrolment.

Type of Stay

Complete and deliver in person to the Registrar's Office upon your arrival and no later than the first week of classes. Please make an appointment to meet with the Registrar by calling 416-922-5474 ext. 221.

International students are required to re-submit this form to the Registrar's Office if they move during their time as a Regis student.
